You're running on Bullseye, when the standard install is now Bookworm (and has been for a year).

dtoverlay=vc4-fkms-v3d is not the default, and we're using dtoverlay=vc4-kms-v3d. Are you totally removing both (which would drop back to software rendering), or leaving one or other.

What framework are you using to generate the graphical content?

Use PrintScreen to capture the desktop to the clipboard and confirm whether it is what is being generated by the app that is incorrect, or whether the display pipeline is messing it up (feels unlikely).

Where do dual monitors come into this?
