Neither of those are a "clean install of the Raspberry Pi OS". It appears you both ran rpi-update, which gets you an unreleased and possibly unreliable kernel.

You are correct, however with a clean install only running `sudo apt update && sudo apt upgrade -y` the fan is also running on 100%.
I also now have a Argo NEO 5 BRED case and the fan is running 100% too.

Here are some pictures, but they port is so small and my camera won't focus it well. I have reseated it multiple times and can't see any bent pins.
Should the fan connector be flat? I pushed it as far as possible without using too much force. I assume it is correct, because the fan is powered up.
