There might be a dirty cache problem somewhere, it is just a 1st guess. You have not provided any other info, so no-one can reproduce exactly.
I have had filesystem corruption on top of an LV. It was an LV from a thinpool exposed to a VM. This happened on both x86-64 and Pi4. All 4 relevant OSses (hosts and guests) Debian12 latest updated, with the exception of Pi4 host Debian12 flavor RPiOS64 Bookworm.
My suspect is that it had to do with trim (pass-through/down or not) but I have not even looked at it as it was very rare (4 times in 18 months or so). I use Btrfs as filesystem, so easy to check and fix on-the-fly without data loss of severe interruption.
Another reason I did not look into detail is that the host x86-64 seemed to have HW troubles and indeed it is now dead. For the Pi4, it uses UAS operated SATA SSD, lots of troubles in the past, so won't spent more time on it. I ordered other brand SBCs that require no extra interface boards or cables, so as long as Pi4 system works, without significant issues, I keep it like that. Else replace and also newer/other OS.
