That you may not have waited long enough during the first boot for all the necessary stuff to have completed.
Try the 32bit lite image.
Let the first boot stuff complete.
To boot from a USB device:
- Format an SD card with FAT32
- Put just bootcode.bin onto it.
- Write your chosen OS to your USB drive
- Insert SD card
- Connect USB drive
- Boot
Lastly, if your Pi needs a full size SD card it's a B not a B+.
