Yes, this too caused a shutdown in about an hour, though I wasn't logging UART at the time. This afternoon I got ~3.5 hours running Home Assistant off SSD. I'll test again with Raspbian on idle and UART logged. To simplify things I suggest installing RPi OS to an SD card, having no peripherals attached and seeing if you get the same behaviour i.e. just leave it at the desktop
I suspect the PMIC chip cluster isn't making sufficient contact with the active cooler given with the acrylic layer case. I had cut off parts of the layer that sits between the cooler and board, but there's still a bit in the lower left corner. That mod improved uptime from minutes to now hours, so maybe it'll get better after this further mod. There doesn't seem to be a temperature sensor for the PMIC. I don't have a thermal camera, but I'm looking to borrow one and confirm.
I'm not using the official case with a fan because reports said the active cooler was more effective. My Pi5 is mounted on a pegboard in a closet and I only wanted minimal protection from bumps, without blocking airflow. It was previously in a Pi2 case with the top and sides removed.
