Are you meaning when you switch the PI off? Does the problem go away if you unplug the HDMI plug or any USB devices that you have connected?

It sounds like it doesn't turn off when the Pi turns on (boots to the os). The Green LED turns on , but the Red Doesn't turn off, so you get Yellow. Sounds like a hardware issue to me.

I'd take a good look at the bottom of the circuit board to make sure its nice and clean with no stray solder blobs etc on it. I'd also take a good look at the top for any missing SMT components.
